Who can enroll in Choice Wireless Lifeline plans?

You may qualify for the Choice Wireless Lifeline or Tribal Lifeline Program if you meet one of the following requirements:

If you get assistance from one or more of the following programs:

  1. Medicaid (Not Medicare)
  2. Supplemental Nutrition Assistance Program (SNAP)
  3. Food Distribution Program on Indian Reservations
  4. Supplemental Security Income (SSI)
  5. Federal Public Housing HUD/Section 8 (FPHA)
  6. Low Income Home Energy Assistance Program (LIHEAP)
  7. National School Lunch (no cost program only)
  8. Temporary Assistance for Needy Families (TANF)
  9. Tribal Lifeline – you may also qualify for these services should you participate in any of the following Tribal programs:
  10. Bureau of Indian Affairs General Assistance
  11. Tribally Administered Temporary Assistance for Needy Families (Tribal TANF)
  12. Head Start (must satisfy income qualifying standard)
  13. Other State mandated programs.

OR, if your total gross annual household income is at or below the federally established poverty levels set forth for the number of persons in your household. (135% for Colorado, Nevada, and Utah, 150% for New Mexico and Arizona.)
